AI Doesn't Have a Hallucination Problem. You Have a Clarity Problem.
What three years of obsessive daily AI use taught me about why your AI gives bad answers.
I told AI I have ADHD.
Within a few days, I noticed every response coming back simplified. Short sentences. Bullet points. Careful, gentle, almost condescending — like I was fragile. Like complexity might break me.
The problem was I’d left AI to guess. “ADHD” is a label with a lot of noise and no real clarity about what I actually mean. All AI could do was make assumptions — and it assumed wrong.
My biggest issue is working memory. I forget where I put things, what I’ve already done, what decisions I already made. I’ll start building something and discover halfway through that I’d already built it a week earlier. No memory of doing it the first time. What I needed from AI was help covering for that — keeping track of where I’m at, what my plan is, where I am in executing it, and keeping it all organized.
So I replaced the label with a description. Here’s what my AI actually reads now, word for word: Working memory is the bottleneck, not intelligence. Forgets where he left off, loses track of sessions. Build systems that remember for him.
Three short sentences of clarity. Dramatically better results.
That sentence, in a lot of ways, became the foundation for my entire ecosystem. Because here’s the thing — AI has the same problem I do. Every time I start a new conversation, AI has a fresh context window and limited memory. It doesn’t remember what we did last time. It doesn’t know where we left off. We both have poor working memories. So we had to build a system outside of any platform — something that could live on its own and anchor us both to where we’re at, what we’ve learned, and what we’ve built. That’s how it all started. One honest description of a limitation, and a system designed around it.
That changed how I think about every bad answer I’ve ever gotten from AI.
The model gets blamed. Every time. AI hallucinated. AI made something up. AI can’t be trusted. I hear it from friends, from clients, from other business owners who tried it twice and gave up.
I’ve been using AI obsessively, every single day, for three years. I run my small business on it. I manage my finances with it. I process therapy insights with it. I’ve built an entire ecosystem of automations that runs while I sleep.
I almost never get so-called hallucinations anymore. And what I’ve learned is this: maybe 99 out of 100 times — maybe all 100 — the model didn’t hallucinate. It gave the human exactly what they asked for, not what they meant.
There’s a big difference between “give me a stat that proves XYZ” and “give me real-world evidence that supports XYZ.” The first one tells AI to find you confirmation — which means it might invent something, because you never told it to find truth. You told it to support what you already believe. The second tells AI to go find what’s actually real. One word changes everything. The model gets judged for failing to read someone’s mind, when the real problem is that the person was never clear about what they wanted.
Sometimes clarity means a few more words — going from a label like “ADHD” to something more descriptive about what you actually need AI to understand. Sometimes it means the opposite — cutting a lot of words and finding the right few.
I built a system to help AI plan my day. Every morning, it looks at all the open tasks in my CRM, reads all the newly inbound communications across every channel — emails, texts, voicemails — checks the voice calls that came in, even scans the random notes I leave on my notepad that I forget to forward to my human assistant. It pulls all of that together and tries to prioritize my day.
The prioritization started simple, but I wanted it to be smart. There are maybe twenty different factors that affect how urgent something is — due dates, yes, but also things that can change urgency regardless of the due date. A new email from a client changes everything. So I built in a learning loop. Every day I’d give it feedback — this should have been higher, that should have been lower — and we kept tweaking the rules. Over time I ended up with 200 lines of logic — weights, decision trees, point values for every factor.
It produced 25 “fires” out of 60. When everything’s a fire, nothing is.
I was complaining about this to a completely different AI conversation — one helping me troubleshoot the process — and it said something that stopped me. “This is old-school rules-based thinking. Why not just let AI make judgments based on context? It just needs to think through the consequences of not doing each task today.”
I scrapped 200 lines and tried one clear direction: sort by consequence to the client first, then to the business. Let AI think.
Five fires. Twelve tasks for today. Four for later. Almost perfect. Haven’t had to tweak it since.
But the inputs aren’t just the prompt. AI also acts on every other piece of information it has access to — documents, files, memories, context from previous conversations. If one sentence in a prompt can shift everything, imagine what a pile of noisy documents can do.
I have a specific build process when working with AI on something new. Fresh chat. Dedicated project. AI loads exactly the information relevant to whatever we’re building. I’m careful not to introduce outside noise.
One time I started a session by saying something like “my brother’s joining this build — let’s build something that shows off what the system can do.” That was the first thing I said, before any clean context had loaded. The rest of the session, I could feel the drift. AI wasn’t optimizing for what the business actually needed. It was optimizing for impressing my brother. My clear goals got diluted by that one sentence — AI decided the point was to show off, not to ship something useful.
I caught it immediately because the output didn’t match what I needed. But it still sounded smart. Still read polished. One sentence of noise, introduced before the real context, was enough to steer the whole build in the wrong direction.
That’s one sentence in one prompt. Now scale it up. Think about every file AI can see.
My Google Drive had 15,135 files. A lot of that mess came from three years of working with AI before the tools were good — downloading files from OpenAI’s canvases, re-uploading them into other conversations and tools, creating duplicates everywhere, different versions with similar names. Dates didn’t help because they’d been copied across so many platforms the timestamps were meaningless. I’m not naturally organized. It piled up.
Every one of those files was an instruction AI was acting on — whether I knew it or not. Outdated drafts, notes I’d forgotten writing, descriptions of projects that no longer existed. AI read all of it and used it to build its understanding of who I am and what I want. And I never noticed, because the output still sounded confident.
I spent a full day with AI triaging every file into three buckets. The first bucket: high signal — accurate, current, trustworthy. These go at the root level of each area of my life, where any AI can find and rely on them. Fewer than a hundred files out of 15,135.
The second bucket: valuable but unverified. Older versions, articles I’d downloaded from the internet, reference material, other people’s work — stuff that might have good information in it but can’t be treated as truth about me and my business. Each area of my life has a folder called a “resource bank,” and all of this goes there. An AI can mine those folders when there’s a specific reason, but it knows not to trust them.
Third bucket: delete.
Then I built an index tracking every file in the ecosystem. Every AI I work with can look at one place and know what’s worth trusting and what needs to be approached carefully. And every night, an automated process scans the entire system for new documents that need triaging and checks my high-signal documents for contradictions. First time I ran that audit, it found fifteen contradictions I didn’t know about. Now it runs every night without me touching it.
Over time I developed what I call North Stars — one dense strategy document per area of my life, written specifically for AI to read. Business, finances, writing, personal growth. When AI reads a North Star, it doesn’t have to guess about my goals or priorities. Instead of scattering information across dozens of conversations and hoping AI pieces it together, I give it one clean document that tells it what it needs to know.
Same pattern across every example. AI reads everything you give it — every document, every memory, every offhand sentence — as an instruction. It executes on those instructions whether they’re accurate or not. And the output always reads polished. That’s the trap.
AI might feel like magic, but it’s deterministic. Your words are code. Every word shifts the instruction slightly.
When your prompts keep getting longer, you’re not getting closer to a good answer. You’re patching bad inputs with more words. Fix the inputs.
Language is code. It always has been — for humans and for AI. It’s how we transfer ideas. And the humans who are going to get the most out of AI aren’t the ones with the best tools or the biggest budgets. They’re the ones who learn to think clearly and communicate that clarity.
Finding clarity and communicating it has always been one of the most valuable skills a person can have. But now the rewards for it are going to compound. Every document you clean up, every description you sharpen, every vague label you replace with something specific — it all feeds into a system that gets better the clearer you get.
When you’re not getting what you want from AI, the answer isn’t to blame the model. It’s to get curious. Be open to the possibility that your side of the conversation could improve. Start with one document. Write down who you are, how you think, and what you actually want — clearly enough that a stranger would understand you. Keep it somewhere you control — not inside any AI platform, but in your own files. Share it with AI at the start of your next conversation.
Here’s what happens: that document never forgets what you taught it. You can plug in any AI — switch platforms tomorrow, try a new model next week — and it instantly inherits the clarity you’ve developed. Your thinking lives in your files, not inside someone else’s system.
